Development of the vertical dimension in human faces results from the interaction of environmental and genetic factors in a mosaic pattern of growth. Discernment and consideration of these factors helps to understand the etiology of an open bite. This informs the treatment planning process and treatment. It also has an impact on retention of the open bite closure. This lecture will review the etiological factors in the development of openbites, and why epigenetic modulation should be a future practice focus.
Dr. James K. Hartsfield, Jr., DMD, MS, MMSc, PhD, FACMGG
Dr. Hartsfield received his DMD from the Medical University of South Carolina, his MS and residency in medical genetics from the Indiana University School of Medicine, his MMSc and orthodontic residency certificate from the Harvard School of Dental Medicine, and his PhD in medical sciences and medical genetics residency from the University of South Florida College of Medicine. He is the E. Preston Hicks Endowed Professor at the University of Kentucky College of Dentistry and holds adjunct or visiting appointments at the Indiana University School of Dentistry, the Indiana University School of Medicine, the University of Illinois at Chicago College of Dentistry, the University of Western Australia Dental School, and the Harvard School of Dental Medicine. He is a diplomate of the American Board of Orthodontics, and the American Board of Medical Genetics and Genomics; and a member and past president of the Angle Society-Midwest component. He practiced orthodontics and medical genetics until retiring from clinical practice and continues research into genetic factors that influence sagittal and vertical development during puberty. Dr. Hartsfield has taught growth and development to orthodontic and pediatric dentistry residents for 30 years.

Learning Objectives:
Through the incipiency of clear aligner treatment, it has been demonstrated that the technique may offer excellent vertical control in the management of hyperdivergent type or anterior open bite malocclusions. Evaluation of techniques used to treat anterior open bites will be discussed in detail, in addition to benefits and detriments of this particular modality. Long-term stability will be evaluated, along with retention considerations.

Dr. Maz Moshiri enjoys working with his father and sister in private practice in St. Louis, MO. When not at work, his passion is to spend time with his wife Michelle, and his son Shaya and daughter Sadira. He is an assistant clinical professor at St. Louis University with a focus on clear aligners, a diplomate of the American Board of Orthodontics, a Fellow in both the American and International College of Dentists, and the Pierre Fauchard Academy. Dr. Maz is also a speaker for Align Technology, and founder of the Aligner Intensive Fellowship LLC, an online clear aligner course that has trained over 4000 orthodontists internationally on clear aligner techniques.

Various sites have been advocated by numerous authors as the “ideal placement site” for min-screw implants (MSI). Many publications have documented the bone quality and density in the more popular interdental sites for MSI placement. The palate, on the other hand, can provide a versatile location that could be used for most anchorage sensitive cases. Dr. Razavi will review a step-by-step protocol in palatal MSI placement and activation protocol for predictable treatment of anterior open bites.
Moe Razavi, DDS, MSD, FRCD(C)
Dr. Moe Razavi received his dental training at Case Western Reserve University. He was awarded his DDS in 2002 and his orthodontic certificate and MSD in 2005. Upon completion of his orthodontic training, he was invited to join the department as an assistant clinical professor, where he founded and directed the Skeletal Anchorage Clinic, and integrated various Temporary Anchorage Device (TAD) Systems into the training program. He is a diplomat of the American Board of Orthodontists, a Fellow of the Royal College of Dentists in Canada, and an ad hoc reviewer for various orthodontic journal publications. Dr. Razavi’s maintains a private practice in Ottawa, Canada.

The evaluation and management of the occlusal planes is sometimes overlooked in orthodontic diagnosis and treatment planning. However orientation of the occlusal planes is often the most significant factor in determining if a specific open bite can be predictably corrected without orthognathic surgery. Through a number of clinical examples Dr. Mair will demonstrate how to identify and treat open bite malocclusions that are amenable to non-surgical correction by way of occlusal plane manipulation.

Dr. Anthony Mair received his dental degree from McGill University in 1987 and his Master of Clinical Dentistry in Orthodontics from University of Western Ontario in 1990. Since then he has been in private practice in Toronto. Dr. Mair is an Adjunct Professor in the Graduate Orthodontics department at Western University and a Clinical Instructor in Graduate Orthodontics at University of Toronto. Dr. Mair has a particular interest in Biomechanics, Treatment Efficiency and Non-surgical Management of complex cases.
